Start with a new image in RGB mode and 
use the marquee tool to draw a rectangle. 
Fill it wit black and save the selection.
Go to SELECT and select none (Ctrl+D)

Apply the stylize/ emboss filter with the 
settings angle -45, height 5 and amount 100.

Apply the filter blur/gaussian blur with 
the settings radius 0,7

Add a new layer and load the selection that 
you saved in step 1. Fill the selection with 
a grey/green shade. Apply render/lightning 
effects with spotlight and texture channel none. 
Here you can play around a little for different 
light results.

Apply the render/lensflare with 50-300mm
and brightness approx. 70.
